Short answer
An order is triggered when the reference quote for the buy or sell side reaches the condition, and in a gap or a fast-moving market it can be filled at the next available price rather than at the price you specified. Check the order type, the Bid/Ask, the trading hours of the symbol and the platform journal, and if something is wrong contact official support with the trade number.
